This paper deals with the malfunctions of the Cyclotron Accelerator System, which was one of the solution proposed for Lead-Bismuth eutectic cooled XADS, and the related design basis initiating events that can be a challenge to the integrity of physical barriers (fuel c1adding and primary system). The FMECA (Failure Mode and Effect and Criticality Analysis) methodology was used for the identification of initiating events, because it allows to make a complete screening of the various causes that may provoke an accident. The quantification of failure rates of cyclotron components has been made referring to available data of fission or fusion plants and information available from vendors, previous studies, existing facilities (LANSCE and PSI accelerators) and literature.
